Clinical Objective

Knee osteoarthritis management must be customized to the structural degeneration stage (Kellgren-Lawrence Grades 1 to 4). Low-impact strengthening, joint lubrication, and activity modification help preserve remaining cartilage, reduce pain, and improve functional stability.

Grade-Wise Protocol

Grades 1 & 2 — Mild Degeneration / Early Intermittent Stiffness

Stationary Cycling (Low Resistance)

Watch

How: Cycle on a stationary bike at low resistance for 10–15 minutes with the seat adjusted so the knee has a slight bend at full extension.

Purpose: Promotes synovial fluid circulation and joint nutrition without high-impact compression.

Straight Leg Raises & Clamshells

Watch

How: Perform standard straight leg raises and side-lying clamshells to build quadriceps and gluteal baseline strength.

Purpose: Stabilizes the lower limb alignment early to prevent asymmetrical joint wear.

Grade 3 — Moderate Degeneration / Frequent Pain & Stiffness

Terminal Knee Extensions (TKEs) with Band

Watch

How: Loop a band behind the knee, step back to create tension, and fully lock out the knee against light resistance.

Purpose: Strengthens the VMO quad muscle to support tracking without deep joint flexion under load.

How: Sit upright in a chair and slowly straighten your leg out in front of you, holding for 3–5 seconds before lowering.

Purpose: Controlled open-chain strengthening tailored to moderate tolerance limits.

Grade 4 — Severe Degeneration / Bone-on-Bone / Chronic Pain

Supine Heel Slides & Gentle Passive ROM

Watch

How: Lying down, gently slide your heel toward and away from your buttocks using a towel or strap assist to remain unloaded.

Purpose: Preserves basic range of motion without bearing bodyweight compressive loads.

Isometric Quad Sets (Bed/Chair)

Watch

How: Sit or lie with legs straight, place a small towel under the knee, and push downward, tightening the thigh muscle for 5 seconds.

Purpose: Maintains muscle activation safely without joint movement in advanced degenerative stages.

Movements & Habits to Avoid

Deep Squats and Deep Knee Bending (> 90 Degrees)

Risk: Dramatically increases patellofemoral and tibiofemoral compressive stress, worsening cartilage breakdown.

High-Impact Running, Jumping, or Pounding Activities

Risk: Exposes degenerate articular surfaces to sharp shock waves and accelerated wear.

Heavy Resisted Open-Chain Knee Extension Machines

Risk: Places massive shear stress across the joint line during terminal extension under heavy loads.

Clinical Guideline

Exercise discomfort should never exceed a mild, tolerable ache (2–3/10). Stop immediately if sharp joint grinding or acute swelling occurs.
