Clinical Objective

Successful ACL rehabilitation focuses on early restoration of full passive knee extension, quadriceps reactivation, gentle neuromuscular control, and later progression to multi-planar stability and dynamic strength. Graft protection is paramount in the early phases.

Recommended Exercises

Prone Hangs (Extension Passive Stretch)

Watch

How: Lie face down on a treatment table or bed with your feet and lower shanks hanging off the edge freely. Relax completely for 3–5 minutes.

Purpose: Restores and maintains full terminal knee extension (hyperextension symmetry), which is critical early on.

Quad Sets with Biofeedback / Towel Roll

Watch

How: Sit or lie with your leg straight. Place a small rolled towel under your heel or knee. Tighten your thigh muscle hard, pushing the back of your knee downward into the surface. Hold for 5–10 seconds.

Purpose: Combats post-operative arthrogenic muscle inhibition and re-educates the quadriceps.

Heel Slides (Active-Assisted Flexion)

Watch

How: Sit or lie on your back with legs straight. Use a strap or your hands to gently pull your heel toward your buttocks, sliding it along the floor or surface, then slowly return.

Purpose: Progressively restores pain-free knee flexion range of motion.

Mini-Squats & Weight Shifts (Closed Chain)

Watch

How: Stand holding onto a secure support or TRX straps. Perform controlled bodyweight mini-squats up to 30–45 degrees of knee flexion, maintaining balanced weight distribution.

Purpose: Safely loads the lower extremity in a closed kinetic chain to build early functional quadriceps and gluteal strength.

Proprioceptive Single-Leg Stances

Watch

How: Once cleared for partial-to-full weight-bearing, stand on the affected leg on a stable floor surface (progressing later to balance pads), keeping posture upright and knee slightly soft.

Purpose: Restores joint position sense, neuromuscular coordination, and dynamic ankle-knee-hip stability.

Movements & Habits to Avoid

Early Open-Chain Knee Extensions (Heavy Terminal Arc)

Risk: Heavy resisted open-chain extension machine exercises between 0 and 45 degrees place direct, unmitigated anterior shear force on the newly healing ACL graft.

Unrestricted Twisting, Pivoting, or Cutting

Risk: Sudden rotational forces or lateral cutting maneuvers before adequate multi-planar graft maturation can cause structural graft failure or re-injury.

Hyperextension Overload / Forced Stretching

Risk: While passive extension is vital, aggressive or forced hyperextension under heavy load can strain the posterior structures and graft fixation sites.

Unprotected High-Impact Plyometrics & Running

Risk: Jumping, deep landing mechanics, and sprinting too early in the timeline bypass critical strength benchmarks and risk severe joint trauma.

Deep Unrestricted Squats / Lunges in Early Phases

Risk: Deep knee flexion angles combined with forward translation under load increase patellofemoral and tibiofemoral joint stress prematurely.

Clinical Guideline
