Clinical Objective
Chondromalacia patella involves the softening of articular cartilage on the underside of the patella. Effective management requires minimizing patellofemoral compressive stress while strengthening the quadriceps (specifically the Vastus Medialis Obliquus) and stabilizing the hips and kinetic chain.
Recommended Exercises
Terminal Knee Extensions (TKEs) with Resistance Band
WatchHow: Loop a resistance band around a sturdy anchor and behind your affected knee. Step back to create tension. Keep your heel on the floor, slightly bend your knee, and straighten your leg fully by squeezing your quad muscle.
Purpose: Isolates the Vastus Medialis Obliquus (VMO) without placing heavy shear stress on the patellofemoral joint.

Straight Leg Raises (SLRs)
WatchHow: Lie on your back with one knee bent (foot flat) and the other leg straight. Tighten your thigh muscle and lift the straight leg about 12 inches off the ground. Hold for 3–5 seconds, then slowly lower.
Purpose: Strengthens the quadriceps safely without bending the knee joint under heavy bodyweight load.

Clamshells
WatchHow: Lie on your side with hips and knees bent at a 45-degree angle, feet stacked. Keeping heels together, raise your top knee as high as possible without rotating your pelvis.
Purpose: Strengthens the gluteus medius, improving lower-limb alignment and reducing inward knee collapse (valgus).

Glute Bridges
WatchHow: Lie on your back with knees bent and feet flat hip-width apart. Squeeze glutes and push through heels to lift hips toward the ceiling until your body forms a straight line.
Purpose: Builds posterior chain strength to properly support overall lower extremity mechanics.

Calf Stretches (Runner’s Stretch)
WatchHow: Stand facing a wall, step one foot back with the heel flat on the floor and leg straight. Lean forward into the wall until a stretch is felt in the calf.
Purpose: Relieves tight calves that can pull on lower leg mechanics and alter kneecap tracking.

Movements & Habits to Avoid
Deep Squats and Heavy Leg Presses
Risk: Squatting past a 90-degree angle dramatically increases compressive forces pushing the kneecap into the femoral groove.
High-Impact Plyometrics & Running on Hard Surfaces
Risk: Jumping, bounding, and high-impact running on concrete create excessive jarring shock across irritated joint surfaces.
Open-Chain Knee Extensions with Heavy Resistance
Risk: Traditional heavy seated knee-extension machine exercises place massive shear stress across the patellar tendon, especially in the terminal 30 degrees.
Forward Lunges
Risk: Shifting heavy dynamic loads directly over a forward-drifting front knee significantly exacerbates patellar tracking friction.
High-Resistance Stair Climbing or Step-Ups
Risk: Steep stepping actions under load can trigger acute flare-ups and sharp patellofemoral pain.
Clinical GuidelineRehabilitation pain should never exceed a mild, dull ache (2–3/10) during exercise. Discontinue any movement that provokes sharp or lingering pain.
